Waiver request (short form) - LDC 2.10.1 - this form allows for up to 2 items

Waiver request (long form) - LDC 2.10.1 - this form allows for up to 14 items - form can be saved and shortened depending on the number of items requested

Waiver request to county municipal services benefit unit (MSBU) - this form is to be used with agricultural lot splits only, per LDC 2.16.1.B(8)(g)

Waiver request to major site plan - this form is to be used when requesting a waiver to major site plan requirements per LDC 2.21.1.A(1)
